The Arc Jefferson - St. Lawrence is one of 35 local Chapters of The Arc New York, the largest provider of supports and services for people intellectual and developmental disabilities in New York state. Community Based Vocational Counselor II
Qualifications - Bachelor’s Degree required. Degree in Human Services or related field preferred. One – two years’ experience working with the developmentally disabled preferred. New York State valid/ acceptable Driver’s License.
Work Authorization/Security Clearance – Authorized to work in the United States.
Summary/Objective - Provide counseling interventions as needed to all people we support in the Career Solutions program. Develop Vocational Plans for ACCES-VR and individuals in ACCES-VR Extended and OMH Extended services. Complete the NYESS Data Entry for all programs as needed.
Essential Functions –
Monitor and track all referrals within the ACCES-VR guidelines.
Maintain quarterly reporting of units for all contracts.
Review and assist with monthly billable services.
Participate in team meetings as scheduled.
Conduct intakes for those supported along with the Employment Specialists.
Provide case file reviews, satisfaction surveys, referrals and other internal documentation.
Review all daily and monthly notes for waiver and non-waiver services.
Conduct employer site visits.
Provide Work Readiness activities to individuals in various contracts.
Complete all admission and discharge documentation for persons supported in the ACCES-VR and OMH Extended programs.
Complete all competitive employment forms as needed.
Provide benefits advisement to referred individuals.
